## Hepamerz (l-ornithine l-aspartate)
### Overview
Hepamerz is a combination of two amino acids, l-ornithine and l-aspartate, used in the management of hyperammonemia, particularly in patients with liver disease. It is thought to promote ammonia detoxification in the liver and peripheral tissues.
### Primary Indications
* Management of latent and manifest hepatic encephalopathy.
* Reduction of elevated blood ammonia levels in patients with cirrhosis.
### Adult Dosing
* **Oral:** Typically 3 to 6 grams (equivalent to 1 to 2 sachets or capsules) three times daily.
* **Intravenous:** Typically 5 grams administered as a slow infusion over 30 minutes to 1 hour, repeated up to three times daily. The total daily dose should not exceed 25 grams.
### Pediatric Dosing
Data for pediatric use is limited. Dosing should be individualized based on the child's weight, clinical condition, and ammonia levels, and typically managed by specialists.
### Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment, as the drug is primarily used in patients with liver disease. However, caution is advised in patients with severe renal impairment.
### Contraindications
* Known hypersensitivity to l-ornithine or l-aspartate.
* Severe renal failure (creatinine clearance < 25 mL/min).
### Adverse Effects
Common adverse effects include gastrointestinal disturbances such as nausea, vomiting, and abdominal pain. Skin rash and pruritus have also been reported.
