## Hepamerz (l-Ornithine l-Aspartate)
### Overview
Hepamerz is a combination of two amino acids, ornithine and aspartate, used in the management of hyperammonemia. It is believed to facilitate the detoxification of ammonia in the liver by promoting the urea cycle and glutamine synthesis.
### Primary Indications
* Treatment of latent and manifest hepatic encephalopathy (HE) associated with chronic liver disease.
### Adult Dosing
* **Oral:** Typically 3 to 6 grams (containing 100 mg ornithine and 167 mg aspartate per gram of powder) three times daily. Maximum dose is generally 18 grams daily.
* **Intravenous (IV):** Varies significantly by formulation and country. Commonly cited doses range from 5 grams to 20 grams infused over 30 minutes to several hours. Follow specific product labeling or local protocol.
### Pediatric Dosing
* Dosing in pediatric populations is not well-established and should be determined by a specialist. Safety and efficacy have not been adequately studied.
### Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment, as the drug aims to support liver function. However, caution is advised in severe hepatic dysfunction.
### Contraindications
