# Hepamerz® (l-Ornithine l-Aspartate)
## Overview
Hepamerz® is a combination of the amino acids l-ornithine and l-aspartate. It is thought to reduce ammonia levels in the body by stimulating the urea cycle and promoting ammonia detoxification in peripheral tissues.
## Primary Indications
Hepatic encephalopathy (HE) in patients with liver cirrhosis.
## Adult Dosing
* **Oral:** Typically 3 grams (which is equivalent to 1 sachet of Hepamerz® 3g) three times daily. The maximum daily dose is generally 9 grams.
* **Intravenous (IV):** Dosing varies, but common regimens include 5 grams to 20 grams per day, often administered as a continuous infusion or divided doses. Specific IV dosing is highly dependent on clinical status and local protocol.
## Pediatric Dosing
Pediatric dosing is not well-established. Use in children should be based on specialist advice and careful risk-benefit assessment.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment, as the drug is metabolized by the liver and its components are excreted by the kidneys. However, caution is advised in severe hepatic impairment.
## Contraindications
* Known hypersensitivity to l-ornithine, l-aspartate, or any component of the formulation.
* Severe kidney disease (uremia).
## Adverse Effects
Common adverse effects include nausea, vomiting, abdominal pain, and diarrhea. Muscle cramps have also been reported.
## Key Drug Interactions
No significant drug interactions are well-documented. However, concurrent use with drugs that affect ammonia levels or urea cycle function should be approached with caution.
## Monitoring
* Monitor for signs and symptoms of hepatic encephalopathy.
