# Dicyclomine (Bentyl)
## Overview
Dicyclomine is a synthetic antispasmodic and anticholinergic agent. It exerts a direct relaxant effect on smooth muscle and inhibits the muscarinic actions of acetylcholine at the postganglionic parasympathetic receptor sites.
## Primary Indications
Functional bowel/irritable bowel syndrome (IBS).
## Adult Dosing
* **Initial:** 20 mg orally every 6 hours.
* **Titration:** May increase to 40 mg orally every 6 hours if tolerated and necessary for efficacy.
* **Maximum:** 160 mg per day. Discontinue if efficacy is not achieved after 2 weeks.
## Pediatric Dosing
* **Safety/Efficacy:** Not established for pediatric use. Use is generally discouraged due to reports of serious respiratory and CNS side effects in infants.
## Dose Adjustments
* **Renal Impairment:** No formal dosage adjustments provided; use with caution.
* **Hepatic Impairment:** Use with caution; monitor for increased anticholinergic effects.

* **Serious:** Anticholinergic toxicity (hallucinations, confusion, tachycardia), urinary retention, and heat prostration (decreased sweating).
## Key Drug Interactions
* **Anticholinergics:** Additive effects (e.g., antihistamines, tricyclic antidepressants) increase risk of toxicity.
* **Potassium Chloride (Solid dosage forms):** Combined use with anticholinergics increases the risk of GI mucosal lesions due to prolonged contact time.
* **CNS Depressants:** May potentiate sedation.
## Monitoring
* Monitor for anticholinergic symptoms (constipation, blurred vision, urinary retention).
* Monitor GI motility and bowel sounds.
* Assess mental status, especially in elderly patients.
## Clinical Pearls
* **Beers Criteria:** Dicyclomine is strongly recommended to be avoided in older adults due to its potent anticholinergic profile.
* **Heat Sensitivity:** Advise patients to avoid extreme exercise or high temperatures, as the drug suppresses perspiration and may lead to heatstroke.
* **Administration:** Ensure patients are aware of potential drowsiness; caution against operating machinery until CNS effects are known.
