## Clobazam (Oral Formulation)
### Overview
Clobazam is a benzodiazepine derivative used as an adjunctive therapy for seizures associated with Lennox-Gastaut Syndrome (LGS). It acts as a central nervous system depressant.
### Primary Indications
* Adjunctive treatment of seizures associated with Lennox-Gastaut Syndrome (LGS) in patients 2 years of age and older.
### Adult Dosing
* **Initiation:** 5 mg twice daily.
* **Titration:** Increase dose by 5 mg every week based on clinical response and tolerability, not to exceed the maximum daily dose.
* **Maintenance:** The usual maintenance dose is 10 mg twice daily (total daily dose of 20 mg).
* **Maximum Daily Dose:** 20 mg twice daily (total daily dose of 40 mg).
### Pediatric Dosing
* **Initiation (2 to less than 10 years of age):** 5 mg once daily.
* **Titration (2 to less than 10 years of age):** Increase dose by 2.5 mg to 5 mg every week based on clinical response and tolerability, not to exceed the maximum daily dose.
* **Maintenance (2 to less than 10 years of age):** The usual maintenance dose is 5 mg twice daily (total daily dose of 10 mg).
* **Maximum Daily Dose (2 to less than 10 years of age):** 10 mg twice daily (total daily dose of 20 mg).
* **Initiation (10 years of age and older, or weighing 30 kg or more):** 5 mg twice daily.
* **Titration (10 years of age and older, or weighing 30 kg or more):** Increase dose by 5 mg every week based on clinical response and tolerability, not to exceed the maximum daily dose.
* **Maintenance (10 years of age and older, or weighing 30 kg or more):** The usual maintenance dose is 10 mg twice daily (total daily dose of 20 mg).
* **Maximum Daily Dose (10 years of age and older, or weighing 30 kg or more):** 20 mg twice daily (total daily dose of 40 mg).
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. Specific dose adjustments are not well-defined, but consider starting at the lower end of the dosing range and titrating slowly.
* **Concomitant Medications:** Dose may need to be adjusted when used with strong CYP3A4 or CYP2C19 inhibitors or inducers. See Key Drug Interactions.
### Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic insufficiency.
### Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, pyrexia, aggression, irritability, cough, pneumonia, upper respiratory tract infection.
* **Serious:** Serious skin reactions (e.g., Stevens-Johnson Syndrome), severe drowsiness, respiratory depression, withdrawal symptoms, suicidal behavior and ideation.
### Key Drug Interactions
* **CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, ritonavir):** May increase clobazam concentrations. Consider dose reduction of clobazam.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease clobazam concentrations. Consider dose increase of clobazam.
* **CYP2C19 Inhibitors (e.g., fluconazole):** May increase clobazam concentrations. Consider dose reduction of clobazam.
* **CNS Depressants (e.g., opioids, alcohol, other sedatives):** Additive CNS depressant effects may occur, leading to profound sedation, respiratory depression, coma, and death. Use together with extreme caution and consider dose reduction of one or both agents.
* **Antiepileptic Drugs (AEDs):** Clobazam can affect concentrations of other AEDs, and vice versa. Monitor AED levels and adjust doses as needed.
### Monitoring
* Monitor for signs of somnolence, fatigue, and respiratory depression, especially after initiation or dose increase.
* Monitor for signs of serious skin reactions.
* Monitor for signs of suicidal behavior or ideation.
* Monitor for withdrawal symptoms if clobazam is discontinued abruptly.
* Monitor liver function tests periodically.
* Monitor for drug interactions, especially with concomitant medications affecting CYP enzymes or other CNS depressants.
### Clinical Pearls
* Clobazam should be used as adjunctive therapy.
* Taper the dose slowly when discontinuing to avoid withdrawal symptoms.
* The risk of serious skin reactions necessitates careful patient monitoring.
* Drowsiness is a common side effect; patients should be cautioned about driving or operating heavy machinery.
* Clobazam is a Schedule IV controlled substance.
