# Clobazam (Oral Formulation)
## Overview
Clobazam is a benzodiazepine with anticonvulsant properties. It is thought to exert its effects by enhancing the activity of the neurotransmitter gamma-aminobutyric acid (GABA).
## Primary Indications
* Adjunctive treatment of seizures associated with Lennox-Gastaut Syndrome (LGS) in patients 2 years of age and older.
* Off-label uses include other types of epilepsy.
## Adult Dosing
* **Lennox-Gastaut Syndrome:** The recommended starting dose is 5 mg twice daily. The dose can be increased by 2.5 mg to 5 mg twice daily every week to a maximum of 20 mg twice daily.
## Pediatric Dosing
* **Lennox-Gastaut Syndrome (2 to 10 years of age):** The recommended starting dose is 2.5 mg twice daily. The dose can be increased by 2.5 mg twice daily every week to a maximum of 10 mg twice daily.
* **Lennox-Gastaut Syndrome (10 years of age and older):** Dosing is the same as for adults.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are established, but consider starting at the lower end of the dosing range.
* **Renal Impairment:** Use with caution. No specific dose adjustments are established.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
## Adverse Effects
Common adverse effects include somnolence, decreased appetite, aggression, irritability, and constipation. Serious adverse effects include severe skin reactions (Stevens-Johnson Syndrome, Toxic Epidermal Necrolysis), and withdrawal symptoms upon abrupt discontinuation.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive sedative effects.
* **CYP2C19 Inhibitors (e.g., fluconazole):** May increase clobazam levels.
* **CYP2C19 Inducers (e.g., rifampin):** May decrease clobazam levels.
* **Other Antiepileptic Drugs (AEDs):** Potential for additive CNS depression. Monitor for efficacy and toxicity.
## Monitoring
* Monitor for therapeutic response (seizure frequency reduction).
* Monitor for adverse effects, especially somnolence, dizziness, and behavioral changes.
* Monitor for signs of skin reactions.
* Monitor for signs of withdrawal if discontinuation is planned.
## Clinical Pearls
* Clobazam is generally initiated at a low dose and titrated slowly to minimize adverse effects.
* Abrupt discontinuation should be avoided due to the risk of withdrawal symptoms. Tapering of the dose is recommended.
* Due to the risk of severe skin reactions, patients should be educated to discontinue the medication and seek immediate medical attention if a rash develops.
* Clobazam is a Schedule IV controlled substance.
