# Clobazam (Oral)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties.
## Primary Indications
Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **Lennox-Gastaut Syndrome (LGS):** The recommended starting dose is 5 mg twice daily. Doses can be increased by 2.5 mg to 5 mg every week to a maximum of 20 mg twice daily.
## Pediatric Dosing
* **Lennox-Gastaut Syndrome (LGS):**
* **2 to <10 years:** The recommended starting dose is 5 mg once daily. Doses can be increased by 2.5 mg to 5 mg every week to a maximum of 10 mg twice daily.
* **≥10 years:** Same as adult dosing.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are recommended, but initiation at lower doses may be prudent.
* **Renal Impairment:** Use with caution. No specific dose adjustments are recommended, but initiation at lower doses may be prudent.
* **Concomitant Medications:** Dose reduction may be necessary if clobazam is used concurrently with strong CYP3A4 inhibitors.
## Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
## Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, fatigue, aggression, irritability, cough.
* **Serious:** Severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), respiratory depression, dependence and withdrawal symptoms, suicidal behavior and ideation.
## Key Drug Interactions
* **CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir):** Increased clobazam levels and toxicity. Consider reducing clobazam dose.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine):** Decreased clobazam levels.
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive CNS depressant effects, increasing risk of sedation and respiratory depression.
* **Other Anticonvulsants:** May alter efficacy or side effect profiles.
## Monitoring
* Monitor for somnolence, cognitive and behavioral changes, and signs of respiratory depression.
* Monitor for signs of severe skin reactions.
* Monitor for signs of withdrawal upon discontinuation.
* Therapeutic drug monitoring may be considered in specific situations, particularly with interacting medications.
## Clinical Pearls
* Clobazam is a schedule IV controlled substance.
* Due to the risk of serious skin reactions, patients should be advised to discontinue clobazam and seek immediate medical attention if a rash develops.
* Gradual tapering is recommended upon discontinuation to avoid withdrawal symptoms.
