# Clobazam (Oral Formulation)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties.
## Primary Indications
Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **LGS:** Initial dose: 5 mg twice daily. Increase by 5 mg to 10 mg every week as needed, not to exceed a total daily dose of 20 mg twice daily.
## Pediatric Dosing (2 to less than 10 years of age, or weighing less than 30 kg)
* **LGS:** Initial dose: 2.5 mg twice daily. Increase by 2.5 mg to 5 mg every week as needed, not to exceed a total daily dose of 10 mg twice daily.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustment recommended, but initiate at the lower end of the dosing range and titrate slowly.
* **Renal Impairment:** Use with caution. No specific dose adjustment recommended, but initiate at the lower end of the dosing range and titrate slowly.
## Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic impairment.
## Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, aggression, irritability, upper respiratory tract infection, pneumonia, ataxia, drooling, vomiting.
* **Serious:** Serious skin reactions (e.g., Stevens-Johnson syndrome), suicidal behavior and ideation, respiratory depression, dependence and withdrawal symptoms, cognitive impairment, behavioral changes.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Increased risk of sedation, respiratory depression, and profound CNS depression.
* **CYP2C19 Inhibitors (e.g., fluconazole):** May increase clobazam plasma concentrations.
* **CYP2C19 Inducers (e.g., rifampin):** May decrease clobazam plasma concentrations.
* **Anticonvulsants:** Monitor for changes in efficacy or toxicity of either agent.
## Monitoring
* Monitor for efficacy in seizure control.
* Monitor for adverse effects, particularly somnolence, behavioral changes, and signs of serious skin reactions.
* Monitor for signs of respiratory depression, especially when used with other CNS depressants.
* Assess for dependence and withdrawal symptoms upon discontinuation.
## Clinical Pearls
* Clobazam should be initiated at a low dose and titrated slowly to minimize adverse effects.
* Patients should be advised not to drive or operate heavy machinery until they know how clobazam affects them.
* Abrupt discontinuation can lead to withdrawal symptoms. Taper the dose gradually when discontinuing.
* Clobazam is a Schedule IV controlled substance.
