## Clobazam (Oral Formulation)
### Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is used as an adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS).
### Primary Indications
* Adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS) in pediatric patients 2 years of age and older.
### Adult Dosing
* **LGS:** Initial dose: 5 mg twice daily. Titrate upwards by 5-10 mg every week as tolerated.
* **Maximum dose:** 20 mg twice daily.
### Pediatric Dosing (2 years and older)
* **LGS:** Initial dose: 5 mg twice daily. Titrate upwards by 5-10 mg every week as tolerated.
* **Maximum dose:** 20 mg twice daily.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustment guidelines are established, but consider a lower starting dose and slower titration.
* **Renal Impairment:** No specific dose adjustment guidelines are established. Monitor for increased adverse effects.
### Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
### Adverse Effects
* **Common:** Somnolence, constipation, decreased appetite, fatigue, aggression, irritability, insomnia, ataxia, vomiting.
* **Serious:** Respiratory depression, somnolence (can be profound and dose-dependent), suicidal behavior and ideation, withdrawal symptoms upon abrupt discontinuation (seizures, tremor, abdominal/muscle cramps, vomiting, sweating, confusion, irritability).
### Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other benzodiazepines, sedating antihistamines):** Additive CNS depressant effects, increasing risk of profound sedation, respiratory depression, coma, and death.
* **CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir):** May increase clobazam plasma concentrations, potentially leading to increased adverse effects.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease clobazam plasma concentrations, potentially leading to reduced efficacy.
* **Other Anticonvulsants:** Clobazam can affect serum concentrations of other antiepileptic drugs (e.g., stiripentol). Monitor for efficacy and toxicity.
### Monitoring
* Monitor for efficacy in seizure control.
* Monitor for signs of somnolence, sedation, and CNS depression.
* Monitor for signs of withdrawal symptoms during dose reduction or discontinuation.
* Monitor for suicidal behavior and ideation.
### Clinical Pearls
* Clobazam can be administered as a single dose at bedtime for those experiencing excessive daytime somnolence.
* Discontinuation should occur gradually with a slow taper to minimize withdrawal symptoms.
* Due to the risk of somnolence and dizziness, patients should be advised to avoid operating heavy machinery or driving until they know how clobazam affects them.
