## Clobazam (Oral Formulation)
### Overview
Clobazam is a benzodiazepine derivative used as an adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS) in pediatric and adult patients. It also has off-label uses in other seizure disorders.
### Primary Indications
* Adjunctive therapy for seizures in Lennox-Gastaut syndrome (LGS).
### Adult Dosing
* **LGS:** Initiate at 5 mg twice daily.
* **Titration:** Increase dose by 5 mg to 10 mg every week as tolerated, not to exceed a maximum of 20 mg twice daily.
* **Alternative initiation:** May be initiated at 5 mg once daily and titrated as above.
### Pediatric Dosing
* **LGS (2 years and older):**
* **Initiation:** 2.5 mg twice daily.
* **Titration:** Increase dose by 2.5 mg to 5 mg every week as tolerated.
* **Maximum dose:** Not to exceed 0.5 mg/kg/day divided into two doses.
* **Note:** Dosing for children younger than 2 years is not well established.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution; no specific dose adjustment guidelines are established. Consider lower doses and slower titration.
* **Renal Impairment:** Use with caution; no specific dose adjustment guidelines are established. Monitor for adverse effects.
### Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
### Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, aggression, irritability, upper respiratory tract infection, vomiting.
* **Serious:** Severe drowsiness, respiratory depression, potential for dependence and withdrawal symptoms upon abrupt discontinuation, increased seizure frequency or status epilepticus (paradoxical effect), suicidal behavior or ideation.
### Key Drug Interactions
* **CNS Depressants (e.g., opioids, alcohol, other sedatives):** Additive CNS depression. Use caution and consider dose reduction of one or both agents.
* **CYP2C19 Inhibitors/Inducers:** Clobazam is a substrate of CYP2C19. Fluconazole (inhibitor) may increase clobazam levels. Rifampin (inducer) may decrease clobazam levels.
* **CNS Stimulants:** May reduce the efficacy of CNS stimulants.
### Monitoring
* Monitor for excessive sedation, respiratory depression, and paradoxical reactions.
* Assess for signs of dependence and withdrawal if discontinuing the medication.
* Monitor for suicidal behavior or ideation.
* Evaluate seizure frequency and control.
### Clinical Pearls
* Clobazam should be tapered gradually upon discontinuation to avoid withdrawal symptoms.
* Due to the risk of somnolence, patients should be advised against operating heavy machinery or driving until they know how clobazam affects them.
* Clobazam can be taken with or without food.
* The active metabolite, N-desmethylclobazam, has a long half-life, contributing to its prolonged effects.
