## Clobazam (Oral Formulation)
### Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties used as adjunctive therapy in certain seizure disorders.
### Primary Indications
Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
### Adult Dosing
* **Lennox-Gastaut Syndrome:**
* Initiate at 5 mg twice daily.
* Increase dose slowly over weeks to a target dose of 0.5 to 1 mg/kg/day, divided into two doses.
* Maximum dose: 20 mg twice daily (40 mg/day).
### Pediatric Dosing (2 years and older)
* **Lennox-Gastaut Syndrome:**
* Initiate at 5 mg twice daily.
* Increase dose slowly over weeks to a target dose of 0.5 to 1 mg/kg/day, divided into two doses.
* Maximum dose: 20 mg twice daily (40 mg/day).
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are recommended, but lower doses may be appropriate.
* **Renal Impairment:** Use with caution. No specific dose adjustments are recommended, but lower doses may be appropriate.
* **Concomitant Medications:** Caution and potential dose reduction of clobazam or concomitant CNS depressants may be necessary due to additive effects.
### Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic insufficiency.
### Adverse Effects
* **Common:** Somnolence, lethargy, drooling, constipation, decreased appetite, ataxia, aggression, irritability, cough.
* **Serious:** Severe skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis), suicidal behavior or ideation, withdrawal symptoms, respiratory depression, paradoxical reactions.
### Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive sedative effects, increased risk of respiratory depression and profound sedation.
* **CYP3A4 Inhibitors/Inducers:** May affect clobazam metabolism. Concomitant use with strong CYP3A4 inhibitors may increase clobazam levels; use with strong CYP3A4 inducers may decrease clobazam levels.
* **Antiepileptic Drugs (AEDs):** Clobazam may affect levels of other AEDs.
### Monitoring
* Monitor for somnolence and sedation, especially at the beginning of treatment and with dose increases.
* Monitor for signs of withdrawal upon discontinuation.
* Monitor for behavioral changes, including suicidal ideation or behavior.
* Monitor for severe skin reactions.
### Clinical Pearls
* Clobazam is typically used as adjunctive therapy and should be part of a comprehensive treatment plan.
* Taper gradually upon discontinuation to avoid withdrawal symptoms.
* Patients should be advised against driving or operating heavy machinery until they know how clobazam affects them.
* The risk of serious skin reactions is higher in the first 8 weeks of treatment.
