# Clobazam (Oral Formulation)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties. It is used as adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS) and other seizure types.
## Primary Indications
Adjunctive therapy for seizures in patients with Lennox-Gastaut syndrome (LGS).
## Adult Dosing
* **LGS:** Initiate at 5 mg twice daily. Increase dose by 5-10 mg every week as needed. The maximum recommended daily dose is 20 mg twice daily.
## Pediatric Dosing
* **LGS (2 years and older):** Initiate at 2.5 mg twice daily. Increase dose by 2.5-5 mg every week as needed. The maximum recommended daily dose is 10 mg twice daily.
* **Dosing for pediatric patients < 2 years is not well-established.**
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dosing adjustments are available, but lower doses may be warranted.
* **Renal Impairment:** Use with caution. No specific dosing adjustments are available.
* **Concomitant Medications:** Consider potential drug interactions (see below).
## Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic impairment.
## Adverse Effects
Common: Somnolence, drooling, constipation, decreased appetite, aggression, ataxia, vomiting.
Serious: Respiratory depression, Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), paradoxical reactions (e.g., agitation, irritability), suicidal behavior/ideation, dependence and withdrawal symptoms.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive CNS depression.
* **CYP3A4 Inhibitors/Inducers:** Clobazam is a substrate of CYP3A4. Inhibitors may increase clobazam levels, and inducers may decrease levels.
* **Other Anticonvulsants:** Monitor for altered efficacy or toxicity.
## Monitoring
* **Clinical Efficacy:** Assess seizure frequency and severity.
* **Adverse Effects:** Monitor for somnolence, behavioral changes, ataxia, respiratory issues, and signs of SJS/TEN.
* **Withdrawal Symptoms:** Monitor for signs of withdrawal upon discontinuation.
## Clinical Pearls
* Clobazam is generally initiated at a low dose and titrated slowly to minimize adverse effects, particularly somnolence and behavioral changes.
* Gradual tapering is essential when discontinuing clobazam to avoid withdrawal symptoms.
* Due to the risk of severe skin reactions (SJS/TEN), patients should be advised to seek immediate medical attention if a rash develops.
* The risk of suicidal behavior or ideation is present with all antiepileptic drugs and requires careful monitoring.
