## Clobazam (Oral Formulation)
### Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is used as an adjunct therapy for seizures associated with Lennox-Gastaut syndrome (LGS) and other seizure types.
### Primary Indications
Adjunctive treatment of seizures in patients with Lennox-Gastaut syndrome (LGS).
### Adult Dosing
* **Initial:** 5 mg twice daily.
* **Titration:** Gradually increase dose to achieve optimal clinical response.
* **Maintenance:** Doses typically range from 5 mg to 15 mg twice daily, not to exceed 20 mg twice daily (40 mg/day).
### Pediatric Dosing
* **Children < 2 years:** Safety and efficacy not established.
* **Children 2 to <10 years:**
* **Initial:** 2.5 mg once daily.
* **Titration:** Gradually increase dose.
* **Maintenance:** Doses typically range from 2.5 mg to 5 mg twice daily.
* **Children ≥ 10 years:** Dosing is the same as for adults.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are recommended, but lower doses may be warranted.
* **Renal Impairment:** Use with caution. No specific dose adjustments are recommended.
### Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic or renal impairment.
### Adverse Effects
Common adverse effects include somnolence, decreased appetite, constipation, aggression, irritability, and gait disturbance. Serious adverse effects include Stevens-Johnson syndrome and toxic epidermal necrolysis, particularly with concomitant use of other anticonvulsants. Withdrawal symptoms can occur.
### Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive CNS depression may occur.
* **CYP3A4 Inhibitors/Inducers:** May alter clobazam levels.
* **Other Anticonvulsants:** Monitor for efficacy and adverse effects; potential for altered clobazam metabolism or increased CNS depression.
### Monitoring
* Monitor for seizure frequency and severity.
* Assess for adverse effects, especially somnolence, behavioral changes, and signs of serious skin reactions.
* Monitor for signs of withdrawal upon discontinuation.
### Clinical Pearls
* Clobazam should be tapered slowly upon discontinuation to avoid withdrawal symptoms.
* Due to its long half-life, it can be dosed once or twice daily.
* There is a risk of serious skin reactions, including Stevens-Johnson syndrome, especially in the initial phase of treatment or during dose changes. Patients should be advised to seek immediate medical attention if a rash develops.
