## Clobazam (Oral Formulation)
### Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is used as adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS).
### Primary Indications
Adjunctive treatment of seizures in patients with Lennox-Gastaut Syndrome (LGS).
### Adult Dosing
* **Initial Dose:** 5 mg twice daily.
* **Titration:** Increase dose by 5 mg to 10 mg every week, as tolerated, to a target dose of 20 mg twice daily.
* **Maximum Dose:** 20 mg twice daily (40 mg per day).
### Pediatric Dosing
* **Children 2 to less than 10 years:**
* **Initial Dose:** 5 mg once daily.
* **Titration:** Increase dose by 2.5 mg to 5 mg every week, as tolerated, to a target dose of 5 mg twice daily.
* **Maximum Dose:** 10 mg twice daily (20 mg per day).
* **Children 10 years or older:** Dosing is the same as adults.
### Dose Adjustments
* **Hepatic Impairment:** Use with caution. Data is limited, but dose reduction may be necessary.
* **Renal Impairment:** Use with caution. Data is limited, but dose reduction may be necessary.
* **Concomitant CYP2C19 Inhibitors:** Dose reduction may be required due to increased exposure.
### Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
### Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, aggression, irritability, pyrexia, gait disturbance, fatigue, vomiting, upper respiratory tract infection, drooling, cough.
* **Serious:** Severe skin reactions (e.g., Stevens-Johnson syndrome), suicidal behavior or ideation, withdrawal symptoms, respiratory depression, dependence, abuse potential.
### Key Drug Interactions
* **CYP2C19 Inhibitors (e.g., fluconazole, omeprazole):** Increased clobazam levels. Consider dose reduction of clobazam.
* **CYP2C19 Inducers (e.g., rifampin, carbamazepine):** Decreased clobazam levels. Consider dose increase of clobazam.
* **CNS Depressants (e.g., alcohol, opioids, other benzodiazepines):** Additive CNS depressant effects.
* **Lacosamide:** Increased risk of adverse events, including somnolence.
* **Valproic acid:** Clobazam may increase valproic acid concentrations; monitor valproic acid levels.
### Monitoring
* Monitor for somnolence and fatigue, especially during dose titration.
* Monitor for signs of suicidal behavior or ideation.
* Monitor for signs of withdrawal upon discontinuation.
* Monitor for respiratory depression, especially when used with other CNS depressants.
* If used with valproic acid, monitor valproic acid serum concentrations.
### Clinical Pearls
* Clobazam is typically used as adjunctive therapy and should be initiated and titrated slowly to minimize adverse effects.
* Withdrawal symptoms can occur upon abrupt discontinuation; taper the dose gradually.
* Due to the risk of severe skin reactions, patients should be advised to seek immediate medical attention if a rash develops.
* Clobazam is a Schedule IV controlled substance.
