# Clobazam (Oral Formulation)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties. It is primarily used as adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS).
## Primary Indications
* Adjunctive therapy for seizures in Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **Initiation:** Typically 5 mg twice daily.
* **Titration:** May be increased by 5-10 mg every 3 days to a maximum of 20 mg twice daily (40 mg/day).
* **Maintenance:** Usual dose is 10-20 mg twice daily. Maximum recommended dose is 20 mg twice daily (40 mg/day).
## Pediatric Dosing
* **Initiation (2 to <10 years):** Typically 5 mg once daily.
* **Titration (2 to <10 years):** May be increased by 2.5-5 mg every 3 days to a maximum of 10 mg twice daily (20 mg/day).
* **Initiation (10 years and older, or weighing 30 kg or more):** Typically 5 mg twice daily.
* **Titration (10 years and older, or weighing 30 kg or more):** May be increased by 5-10 mg every 3 days to a maximum of 20 mg twice daily (40 mg/day).
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are established, but lower doses may be warranted.
* **Renal Impairment:** Use with caution. No specific dose adjustments are established.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
## Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, aggression, irritability, upper respiratory tract infection, fatigue, rash.
* **Serious:** Suicidal behavior and ideation, severe skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis), respiratory depression, dependence and withdrawal symptoms, behavioral changes.
## Key Drug Interactions
* **CYP2C19 Inhibitors (e.g., fluconazole, omeprazole):** May increase clobazam concentrations. Consider dose reduction of clobazam.
* **CYP2C19 Inducers (e.g., rifampin):** May decrease clobazam concentrations. Consider dose increase of clobazam.
* **CNS Depressants (e.g., alcohol, opioids, other benzodiazepines):** Additive CNS depressant effects. Increased risk of sedation, respiratory depression.
* **Phenobarbital, Phenytoin, Primidone:** Clobazam can increase serum concentrations of these antiepileptic drugs. Monitor AED levels and watch for toxicity.
## Monitoring
* Monitor for signs of suicidal behavior or ideation.
* Monitor for respiratory depression, especially with concomitant use of other CNS depressants.
* Monitor for signs of withdrawal if discontinuing therapy.
* Monitor for rash and discontinue immediately if suspected.
* Therapeutic drug monitoring of clobazam is generally not required but may be considered in specific situations.
## Clinical Pearls
* Clobazam is intended for adjunctive therapy; do not use as monotherapy for epilepsy.
* Due to the risk of serious skin reactions, instruct patients to discontinue clobazam and seek immediate medical attention if a rash develops.
* Gradual dose reduction is recommended upon discontinuation to avoid withdrawal symptoms.
* Clobazam can cause significant somnolence; patients should be cautioned about operating machinery or driving.
