## Clobazam (Oral Formulation)
### Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties. It is a Schedule IV controlled substance.
### Primary Indications
* Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
### Adult Dosing
* **Initiation:** Typically 5 mg twice daily.
* **Maintenance:** Can be titrated up to 10 mg twice daily.
* **Maximum:** 20 mg twice daily.
### Pediatric Dosing (2 years and older)
* **Initiation:** 5 mg twice daily.
* **Maintenance:** Can be titrated up to 10 mg twice daily.
* **Maximum:** 20 mg twice daily.

### Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustment guidance is established, but consider lower doses due to potential for increased exposure.

### Contraindications
* Hypersensitivity to clobazam or other benzodiazepines.
* Severe respiratory insufficiency.

### Adverse Effects
* **Common:** Somnolence, sedation, fatigue, increased appetite, constipation, drooling, respiratory infections, irritability, aggression, difficulty with coordination.
* **Serious:** Suicidal behavior and ideation, severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), respiratory depression, dependence and withdrawal symptoms, behavioral changes.
### Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other benzodiazepines, sedating antihistamines):** Additive CNS depressant effects, increasing risk of sedation, respiratory depression, and coma.
* **CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir):** May increase clobazam plasma concentrations.
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine):** May decrease clobazam plasma concentrations.
* **Central Nervous System Stimulants:** Clobazam may reduce the efficacy of stimulants.
### Monitoring
* Monitor for clinical effectiveness in seizure control.
* Monitor for adverse effects, particularly somnolence, behavioral changes, and signs of respiratory depression.
* Monitor for signs of suicidal behavior or ideation.
* Assess for signs of tolerance or dependence, especially with long-term use or abrupt discontinuation.
### Clinical Pearls
* Clobazam is generally considered to have a lower risk of sedation compared to some other benzodiazepines, but somnolence remains a common side effect.
* Abrupt discontinuation can lead to withdrawal symptoms. Tapering is recommended.
* Due to its long half-life, clobazam can accumulate, especially in elderly patients or those with hepatic impairment.
