# Clobazam (Oral Formulation)
## Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is primarily used as adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS).
## Primary Indications
* Adjunctive treatment of seizures in patients with Lennox-Gastaut syndrome (LGS).
## Adult Dosing
* **Initiation:** 5 mg twice daily.
* **Titration:** Increase dose by 5 mg every week to a target dose of 20 mg twice daily.
* **Maximum Dose:** 20 mg twice daily (40 mg/day).
## Pediatric Dosing
* **Initiation (2 years and older):** 5 mg once daily.
* **Titration (2 years and older):** Increase dose by 5 mg every week. Doses should be administered once or twice daily depending on patient response and tolerability.
* **Maximum Dose (2 years and older):** 20 mg twice daily (40 mg/day).
* **Dosing for patients younger than 2 years:** Dosing for this population is not well-established and should be individualized based on clinical response and tolerability.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. Dose reduction may be necessary; specific guidelines are limited.

## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
## Adverse Effects
Common adverse effects include somnolence, decreased appetite, constipation, aggression, irritability, and respiratory tract infections. Serious adverse effects can include severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), withdrawal symptoms, and suicidal behavior or ideation.
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Additive CNS depression.
* **CYP2C19 Inhibitors/Inducers:** Clobazam is a substrate of CYP2C19. Inhibitors may increase clobazam levels, and inducers may decrease levels.
* **Perampanel:** May increase clobazam levels.
## Monitoring
* Monitor for efficacy in seizure control.
* Monitor for adverse effects, particularly somnolence, behavioral changes, and severe skin reactions.
* Monitor for signs and symptoms of withdrawal if discontinued.
## Clinical Pearls
* Clobazam should be tapered gradually when discontinuing to avoid withdrawal symptoms.
* Risk of severe skin reactions is a significant concern; patients should be advised to seek immediate medical attention for any new rash.
