# Clobazam (Oral Formulation)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties. It acts as a positive allosteric modulator of GABA$_{A}$ receptors, enhancing inhibitory neurotransmission.
## Primary Indications
* Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
* Off-label use for other seizure types and anxiety disorders.
## Adult Dosing
* **LGS:** Initial dose is 5 mg twice daily. May increase by 5 mg to 10 mg every week.
* **Maximum Dose:** 20 mg twice daily (40 mg/day).
## Pediatric Dosing (2 to less than 10 years of age OR weighing less than 30 kg)
* **LGS:** Initial dose is 5 mg once daily. May increase by 2.5 mg to 5 mg every week.
* **Maximum Dose:** 10 mg twice daily (20 mg/day).
*Note: Dosing for pediatric patients 10 years of age and older OR weighing 30 kg and above should follow adult dosing guidelines.*
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustment guidelines are established, but lower doses may be appropriate.
* **Renal Impairment:** Use with caution. No specific dose adjustment guidelines are established, but lower doses may be appropriate.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
* Severe respiratory impairment.
## Adverse Effects
Common adverse effects include somnolence, decreased appetite, constipation, aggression, irritability, upper respiratory tract infection, and falls. Serious adverse effects include severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), respiratory depression, dependence, and withdrawal symptoms.
## Key Drug Interactions
* **CYP3A4 Inducers/Inhibitors:** Clobazam is metabolized by CYP2C19 and CYP3A4. Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) may increase clobazam levels. Strong CYP3A4 inducers (e.g., rifampin, carbamazepine) may decrease clobazam levels.
* **CNS Depressants:** Additive CNS depression may occur with alcohol, opioids, other benzodiazepines, and other sedating medications.
* **Antiepileptic Drugs (AEDs):** Clobazam may increase serum concentrations of certain AEDs (e.g., stiripentol, valproic acid).
## Monitoring
* Monitor for efficacy (seizure frequency reduction).
* Monitor for adverse effects, especially somnolence, cognitive impairment, and behavioral changes.
* Monitor for signs of respiratory depression, particularly when used with other CNS depressants.
* Monitor for signs of severe skin reactions.
* Consider monitoring drug levels if efficacy is poor or toxicity is suspected, especially in conjunction with other interacting medications.
## Clinical Pearls
* Clobazam should be used as adjunctive therapy.
* Taper the dose gradually when discontinuing to minimize withdrawal symptoms.
* Patients and caregivers should be advised about the potential for somnolence and to avoid activities requiring mental alertness until the drug's effect is known.
* Due to the risk of severe skin reactions, patients should be instructed to discontinue clobazam and seek immediate medical attention if a rash develops.
