# Clobazam (Oral Formulation)
## Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is thought to exert its effects by enhancing the inhibitory effects of gamma-aminobutyric acid (GABA).
## Primary Indications
* Adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **LGS:** The recommended starting dose is 5 mg twice daily.
* The dose should be titrated upward by 5 mg to 10 mg every week based on clinical response and tolerability.
* The maximum recommended dose is 20 mg twice daily.
## Pediatric Dosing
* **LGS (2 to less than 10 years of age):**
* Starting dose: 2.5 mg twice daily.
* Titration: Increase by 2.5 mg to 5 mg every week based on clinical response and tolerability.
* Maximum recommended dose: 10 mg twice daily.
* **LGS (10 years of age and older):** Dosing is the same as for adults.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. No specific dose adjustments are provided in the prescribing information, but reduced doses may be necessary due to decreased metabolism.
* **Renal Impairment:** Use with caution. No specific dose adjustments are provided in the prescribing information.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
## Adverse Effects
* **Common:** Somnolence, decreased appetite, constipation, fatigue, aggression, irritability, pyrexia, respiratory tract infections, pneumonia.
* **Serious:** Suicidal behavior and ideation, severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), respiratory depression, dependence and withdrawal symptoms, paradoxical reactions (increased seizure frequency/intensity, excitation).
## Key Drug Interactions
* **CYP2C19 Inhibitors (e.g., fluconazole, omeprazole):** May increase clobazam concentrations. Consider dose reduction of clobazam.
* **CYP2C19 Inducers (e.g., rifampin, carbamazepine):** May decrease clobazam concentrations. Consider dose increase of clobazam.
* **CNS Depressants (e.g., opioids, alcohol, other sedatives):** Additive CNS depressant effects may occur, increasing the risk of sedation, respiratory depression, and potentially fatal outcomes.
## Monitoring
* Monitor for signs of suicidal behavior and ideation.
* Monitor for signs of severe skin reactions.
* Monitor for drowsiness and sedation, especially when co-administered with other CNS depressants.
* Monitor for effectiveness in seizure control.
* Monitor for signs of withdrawal upon discontinuation.
## Clinical Pearls
* Clobazam should be initiated by a physician experienced in the treatment of epilepsy.
* Gradual withdrawal is recommended to avoid withdrawal symptoms.
* Due to its long half-life, clobazam can accumulate with chronic use.
* Risk of serious skin reactions is higher with concomitant use of certain antiepileptic drugs.
