# Clobazam (oral formulation)
## Overview
Clobazam is a 1,5-benzodiazepine with anticonvulsant properties. It is thought to exert its effects by enhancing the activity of the inhibitory neurotransmitter GABA.
## Primary Indications
Adjunctive treatment for Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **Initiation:** 5 mg twice daily.
* **Titration:** Increase by 5 mg to 10 mg every week to a target dose of 20 mg twice daily.
* **Maximum Dose:** 20 mg twice daily (40 mg/day).
## Pediatric Dosing (2 years and older)
* **Initiation:** 5 mg twice daily.
* **Titration:** Increase by 5 mg to 10 mg every week to a target dose of 10 mg twice daily.
* **Maximum Dose:** 20 mg twice daily (40 mg/day). Dosing should be individualized.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution; consider lower doses.
* **Concomitant CYP2C19 Inhibitors:** May require dose reduction of clobazam.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic impairment.
## Adverse Effects
* **Common:** Somnolence, aggression, irritability, constipation, decreased appetite, ataxia, pyrexia, pneumonia.
* **Serious:** Serious skin reactions (e.g., Stevens-Johnson syndrome), respiratory depression, suicidal behavior and ideation, withdrawal symptoms upon discontinuation.
## Key Drug Interactions
* **CYP2C19 Inhibitors (e.g., fluconazole):** May increase clobazam levels and risk of adverse effects.
* **CYP2C19 Inducers (e.g., rifampin):** May decrease clobazam levels.
* **CNS Depressants (e.g., opioids, alcohol):** Additive sedative effects and increased risk of respiratory depression.
* **Other Anticonvulsants:** Potential for additive CNS depression or altered efficacy.
## Monitoring
* Monitor for somnolence, behavioral changes, and signs of respiratory depression.
* Assess for efficacy and tolerability.
* Monitor for signs of withdrawal symptoms if discontinuing.
* Consider monitoring for serious skin reactions.
## Clinical Pearls
* Clobazam is associated with a risk of serious skin reactions; patients should be advised to seek immediate medical attention if a rash develops.
* Due to the risk of somnolence, patients should be cautioned about driving or operating heavy machinery until they know how clobazam affects them.
* Abrupt discontinuation can lead to withdrawal symptoms; gradual tapering is recommended.
* Clobazam is a Schedule IV controlled substance.
