# Chymosin forte
## Overview
Chymosin forte is a combination product containing chymotrypsin and trypsin, proteolytic enzymes derived from animal sources. It is intended for enzymatic debridement and reduction of inflammation and edema.
## Primary Indications
* Adjunctive treatment for inflammation and edema associated with:
* Traumatic injuries (e.g., sprains, strains, contusions)
* Post-operative swelling
* Abscesses, cellulitis
* Haematomas
## Adult Dosing
The typical adult dose is 1-2 tablets (containing a specified amount of chymotrypsin and trypsin, typically 50,000 to 100,000 Units of proteolytic activity) administered orally, 3 to 4 times daily. The exact number of tablets and frequency may depend on the specific formulation and severity of the condition. Tablets should be swallowed whole without chewing.
## Pediatric Dosing
There is no established pediatric dosing for chymosin forte. Its use in children is generally not recommended due to lack of safety and efficacy data.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment. However, caution may be warranted in patients with severe dysfunction.
## Contraindications
* Known hypersensitivity to chymotrypsin, trypsin, or any component of the formulation.
* Use with caution in patients with impaired blood clotting or those receiving anticoagulant therapy.
## Adverse Effects
* Gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal discomfort).
* Allergic reactions (rash, itching, swelling, difficulty breathing) are rare but possible.
* Changes in stool odor or color may occur.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin, heparin, direct oral anticoagulants):** Concomitant use may theoretically increase the risk of bleeding. Monitor closely for signs of hemorrhage.
* **Antibiotics:** Some sources suggest potential for improved penetration of antibiotics into infected tissues, though this is not definitively established.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Monitor for gastrointestinal side effects.
* In patients with bleeding risks or on anticoagulants, monitor for signs of bleeding.
## Clinical Pearls
* Chymosin forte is best taken on an empty stomach (e.g., 1 hour before or 2 hours after meals) to maximize absorption and enzymatic activity.
* It is an adjunct therapy and should not replace standard medical treatment for infections or severe injuries.
* The effectiveness and safety in specific patient populations (e.g., elderly, pregnant or breastfeeding women) are not well-established.
