# Chymosin forte
## Overview
Chymosin forte is a combination product containing chymotrypsin and trypsin, proteolytic enzymes.
## Primary Indications
Chymosin forte is indicated for the management of inflammation and edema associated with certain medical conditions, such as postoperative swelling, bruising, and inflammatory joint conditions. It is also used to promote wound healing.
## Adult Dosing
The typical adult dose is one tablet (containing chymotrypsin and trypsin, typically 50,000 to 100,000 A.U. combined or per component) administered orally every 6 to 8 hours, usually on an empty stomach to optimize absorption. Specific dosing depends on the product formulation and severity of the condition.
## Pediatric Dosing
There is no established pediatric dosing for chymosin forte. Its use in children is generally not recommended.
## Dose Adjustments
Dose adjustments are not typically required based on renal or hepatic impairment, as the drug is absorbed systemically and metabolized. However, use with caution in patients with severe impairment.
## Contraindications
* Known hypersensitivity to chymotrypsin, trypsin, or any component of the formulation.
* Active infection or conditions where there is a risk of systemic spread of infection.
## Adverse Effects
* Gastrointestinal disturbances (nausea, vomiting, diarrhea).
* Allergic reactions (rash, itching, hives).
* Dizziness or headache.
## Key Drug Interactions
* **Anticoagulants/Antiplatelets:** Theoretically, concurrent use with anticoagulants (e.g., warfarin) or antiplatelet agents (e.g., aspirin, clopidogrel) may increase the risk of bleeding due to proteolytic activity, though clinical significance is not well-established.
* **Antibiotics:** Some sources suggest it may enhance antibiotic penetration to inflamed tissues, but this is not a direct pharmacokinetic interaction.
## Monitoring
* Monitor for signs of allergic reaction.
* Monitor for gastrointestinal adverse effects.
* Assess for clinical improvement in inflammation and pain.
## Clinical Pearls
* Administer on an empty stomach (at least 30 minutes before or 2 hours after meals) for optimal absorption.
* The efficacy of chymosin forte is debated in some medical communities, and its use may vary based on local protocols and physician preference.
* It is not a substitute for standard treatments of underlying conditions.
