# Attapulgite
## Overview
Attapulgite is a naturally occurring hydrated magnesium aluminum silicate clay with adsorbent properties.
## Primary Indications
Attapulgite is indicated for the symptomatic relief of diarrhea.
## Adult Dosing
* **Oral Suspension:** 12 grams (typically 240 mL of a 0.5 g/mL suspension) may be given for the first bowel movement, followed by 6 grams (typically 120 mL) after each subsequent bowel movement, not to exceed 48 grams per day.
* **Tablets:** 2 to 4 tablets (750 mg each) taken at the onset of diarrhea, followed by 2 tablets after each subsequent bowel movement. Do not exceed 8 tablets in 24 hours.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established. Consult specific pediatric guidelines or a pediatrician.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to attapulgite.
* History of intestinal obstruction.
## Adverse Effects
Constipation is the most common adverse effect. Other potential effects include abdominal pain, nausea, and vomiting.
## Key Drug Interactions
Attapulgite can adsorb other medications, potentially decreasing their absorption and efficacy. Administer other oral medications at least 2 hours before or 4 hours after attapulgite.
## Monitoring
* Monitor for signs of constipation or intestinal obstruction.
* Assess effectiveness in controlling diarrhea.
## Clinical Pearls
* Shake oral suspensions well before use.
* If diarrhea does not improve within 48 hours, or if fever or blood is present in the stool, discontinue use and consult a healthcare provider.
