## Overview
Attapulgite is a naturally occurring hydrated magnesium aluminum silicate used as an antidiarrheal agent. It works by adsorcing water and toxins in the gastrointestinal tract, which can help to solidify stools and reduce the frequency of bowel movements.
## Primary Indications
* Symptomatic treatment of diarrhea.
## Adult Dosing
* **For suspension:** 15 mL to 30 mL every 3 to 6 hours as needed.
* **For tablets:** 2 tablets initially, followed by 1 to 2 tablets after each loose bowel movement. Do not exceed 12 tablets in 24 hours.
* Dosing may vary based on specific product formulation and severity of symptoms.
## Pediatric Dosing
Dosing for pediatric patients is not well-established and should be based on product labeling or local protocol. Generally, lower doses are recommended for children, and it is crucial to rule out more serious causes of diarrhea in this population.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to attapulgite.
* Should not be used in patients with severe abdominal pain, nausea, vomiting, or fever, or in patients with suspected intestinal obstruction, as these may indicate more serious conditions requiring alternative management.
## Adverse Effects
Attapulgite is generally considered safe with a low incidence of adverse effects. Potential adverse effects include:
* Constipation (most common)
* Abdominal discomfort
* Nausea
* Vomiting
## Key Drug Interactions
Attapulgite may adsorb other medications, potentially decreasing their absorption and efficacy. It is recommended to administer other oral medications at least 2 hours before or after attapulgite.
## Monitoring
* Monitor for effectiveness (resolution of diarrhea).
* Monitor for signs of constipation or other adverse effects.
* Assess for dehydration, especially in pediatric patients or those with severe diarrhea.
## Clinical Pearls
* Ensure adequate hydration, especially if constipation occurs.
* If diarrhea persists for more than 48 hours, or if fever or bloody stools are present, discontinue use and seek medical attention.
* When used concurrently with other oral medications, separate administration by at least 2 hours to minimize drug interactions.
