# Attapulgite
## Overview
Attapulgite is a naturally occurring hydrated magnesium aluminum silicate clay used as an antidiarrheal agent. It works by adsorbing water and toxins in the gastrointestinal tract, thereby increasing stool consistency and reducing transit time.
## Primary Indications
* Symptomatic treatment of acute nonspecific diarrhea.
## Adult Dosing
* **Oral Suspension:** 3.0 to 6.0 grams initially, followed by 1.5 to 3.0 grams after each bowel movement. Maximum daily dose: 18 grams.
* **Tablets:** 2.0 grams initially, followed by 1.0 gram after each bowel movement. Maximum daily dose: 10 grams.
## Pediatric Dosing
* **Children 6 to 12 years:** 1.5 to 3.0 grams orally every 4 hours as needed. Maximum daily dose: 9 grams.
* **Children 3 to 6 years:** 0.75 to 1.5 grams orally every 4 hours as needed. Maximum daily dose: 4.5 grams.
* **Children under 3 years:** Consult a pediatrician. Safety and efficacy are not established.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to attapulgite.
* Not recommended for treating diarrhea associated with antibiotic use or Clostridium difficile infection.
## Adverse Effects
* Constipation is the most common adverse effect.
* Abdominal discomfort, nausea, and vomiting may occur.
## Key Drug Interactions
Attapulgite can adsorb other medications, potentially decreasing their absorption and efficacy.
* Administer other oral medications at least 2 hours before or after attapulgite.
* Particular caution with digoxin, lincomycin, tetracyclines, and oral contraceptives.
## Monitoring
* Monitor for improvement in diarrhea symptoms.
* Assess for signs of constipation.
* Evaluate for potential drug interactions if patient is taking other oral medications.
## Clinical Pearls
* Attapulgite is a bulk-forming agent and should be used for short-term symptomatic relief.
* It is important to rule out more serious causes of diarrhea, especially if accompanied by fever, bloody stools, or severe abdominal pain.
* Ensure adequate hydration, particularly in children.
