# Attapulgite Drugs
## Overview
Attapulgite is a naturally occurring complex clay mineral used as an absorbent and antidiarrheal agent. It works by adsorbing excess water and toxins in the gastrointestinal tract.
## Primary Indications
* Symptomatic relief of diarrhea.
* Adjunct in the treatment of nausea, vomiting, and abdominal discomfort associated with diarrhea.
## Adult Dosing
* **Suspension:** 15 mL to 30 mL every 3 hours as needed, up to 6 doses in 24 hours.
* **Tablets:** 2 tablets initially, then 1 to 2 tablets after each bowel movement as needed, not to exceed 12 tablets in 24 hours.
## Pediatric Dosing
* **Children 6 to 12 years:**
* **Suspension:** 15 mL every 3 hours as needed, up to 6 doses in 24 hours.
* **Tablets:** 1 tablet every 3 hours as needed, not to exceed 6 tablets in 24 hours.
* **Children 3 to 6 years:**
* **Suspension:** 7.5 mL every 3 hours as needed, up to 6 doses in 24 hours.
* **Tablets:** Not generally recommended for this age group due to choking hazard.
* **Children under 3 years:** Consult a healthcare professional. Dosing is not well-established and risk of constipation needs careful consideration.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. However, use with caution in patients with a history of constipation.
## Contraindications
* Hypersensitivity to attapulgite.
* Intestinal obstruction.
* Severe abdominal pain, fever, or bloody/mucus in the stool, which may indicate a more serious condition requiring medical evaluation.
## Adverse Effects
The most common adverse effect is constipation. Other potential effects include nausea, vomiting, and abdominal cramping.
## Key Drug Interactions
Attapulgite can decrease the absorption of many orally administered drugs by binding to them in the gastrointestinal tract.
* **Recommendation:** Separate administration of attapulgite from other medications by at least 2 hours (preferably 3-4 hours).
## Monitoring
* Monitor for effectiveness in controlling diarrhea.
* Assess for signs of constipation, especially with prolonged use or in children.
## Clinical Pearls
* Shake suspension well before use.
* Do not use concurrently with other medications without consulting a healthcare professional due to potential absorption interference.
* If diarrhea persists for more than 48 hours, or if fever or bloody stools develop, discontinue use and seek medical attention.
* Adequate fluid intake is crucial to prevent dehydration and manage potential constipation.
