# Attapulgite
## Overview
Attapulgite is a naturally occurring hydrated magnesium aluminum silicate clay that acts as an adsorbent. It is primarily used for its gastrointestinal adsorbent properties.
## Primary Indications
* Symptomatic treatment of acute diarrhea.
## Adult Dosing
* **Oral Suspension:** 3 grams to 6 grams every 3 hours as needed, not to exceed 18 grams per 24 hours.
* **Tablets:** 1 gram to 2 grams every 3 hours as needed, not to exceed 6 grams per 24 hours.
## Pediatric Dosing
* **Children 6 to 12 years:**
* Oral Suspension: 1.5 grams to 3 grams every 3 hours as needed, not to exceed 9 grams per 24 hours.
* Tablets: 1 gram every 3 hours as needed, not to exceed 3 grams per 24 hours.
* **Children 3 to 6 years:**
* Oral Suspension: 0.75 grams to 1.5 grams every 3 hours as needed, not to exceed 4.5 grams per 24 hours.
* Tablets: Use oral suspension or consult a pediatrician for appropriate tablet dosing.
* **Children under 3 years:** Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to attapulgite.
* Patients with severe abdominal pain, nausea, vomiting, or fever, unless directed by a physician, as these may be signs of serious conditions like appendicitis or intestinal obstruction.
## Adverse Effects
Generally well-tolerated. May include constipation, abdominal cramping, or diarrhea in some individuals.
## Key Drug Interactions
Attapulgite can adsorb other medications, potentially reducing their absorption and efficacy. Separate administration of other oral medications by at least 2 hours before or after attapulgite.
## Monitoring
* Monitor for symptomatic improvement of diarrhea.
* Assess for signs of constipation.
* Evaluate for dehydration, especially in pediatric patients.
## Clinical Pearls
* Attapulgite should be used for short-term symptomatic relief of diarrhea.
* If diarrhea persists for more than 48 hours or is accompanied by fever or blood in the stool, discontinue use and consult a healthcare provider.
* Ensure adequate hydration, especially in children, as diarrhea can lead to dehydration.
