# Attapulgite
## Overview
Attapulgite is an adsorbent used for symptomatic treatment of diarrhea. It works by absorbing excess water and toxins in the gastrointestinal tract.
## Primary Indications
* Symptomatic treatment of acute diarrhea.
## Adult Dosing
* **Oral suspension:** 1.5 to 3 grams (approximately 10 to 20 mL) every 1 to 3 hours as needed, up to a maximum of 21 grams per day.
* **Tablets:** 1 gram (2 tablets) initially, followed by 1 gram (2 tablets) after each bowel movement, up to a maximum of 8 grams per day.
## Pediatric Dosing
Dosing for children is not well-established. Use with caution and consult specific pediatric guidelines or a pediatrician.
## Dose Adjustments
No specific dose adjustments are recommended for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to attapulgite.
* Should not be used in patients with severe abdominal pain, nausea, vomiting, or fever of unknown origin without medical evaluation.
## Adverse Effects
Constipation is the most common adverse effect.
## Key Drug Interactions
* **Decreased absorption of other medications:** Due to its adsorbent properties, attapulgite may reduce the absorption of other orally administered drugs. Separate administration by at least 2 hours.
## Monitoring
* Monitor for resolution of diarrhea symptoms.
* Monitor for constipation.
## Clinical Pearls
* Attapulgite is a mineral-based antidiarrheal and is not absorbed systemically.
* Ensure adequate hydration, especially in children and the elderly, as diarrhea can lead to dehydration.
* If diarrhea persists beyond 48 hours or is accompanied by fever or bloody stools, medical attention is warranted.
