## Attapulgite
### Overview
Attapulgite is a natural clay adsorbent used for the symptomatic treatment of diarrhea. It works by absorbing excess water and toxins in the gastrointestinal tract, leading to firmer stools.
### Primary Indications
* Symptomatic relief of acute nonspecific diarrhea.
### Adult Dosing
* **Oral suspension:** 750 mg to 1500 mg (10 mL to 20 mL) every 3-4 hours as needed, not to exceed 7 doses (10500 mg or 140 mL) in 24 hours.
* **Tablets:** 1500 mg to 3000 mg (2 to 4 tablets) every 3-4 hours as needed, not to exceed 7 doses (10500 mg or 28 tablets) in 24 hours.
### Pediatric Dosing
* **Children 6-12 years:**
* Oral suspension: 750 mg (10 mL) every 3-4 hours as needed, not to exceed 7 doses (7500 mg or 70 mL) in 24 hours.
* Tablets: 1500 mg (2 tablets) every 3-4 hours as needed, not to exceed 7 doses (10500 mg or 14 tablets) in 24 hours.
* **Children 3-6 years:**
* Oral suspension: 750 mg (10 mL) every 3-4 hours as needed, not to exceed 5 doses (5000 mg or 50 mL) in 24 hours.
* **Children under 3 years:** Safety and efficacy not established. Consult pediatrician.
### Dose Adjustments
No dose adjustment is typically required for hepatic or renal impairment, as systemic absorption is minimal. However, dose may need to be adjusted based on severity of symptoms and response.
### Contraindications
* Hypersensitivity to attapulgite.
* History of abdominal pain, nausea, vomiting, or fever associated with diarrhea, as these may indicate a more serious underlying condition requiring medical evaluation.
### Adverse Effects
Constipation is the most common adverse effect. Other potential effects include abdominal discomfort and fecal impaction, particularly with excessive use.
### Key Drug Interactions
Attapulgite can adsorb other oral medications, potentially reducing their absorption and efficacy.
* Separate administration of attapulgite from other oral medications by at least 2 hours (preferably 4 hours).
### Monitoring
* Monitor for relief of diarrhea symptoms (frequency and consistency of stools).
* Monitor for signs of constipation or fecal impaction.
* Assess for improvement in abdominal discomfort.
### Clinical Pearls
* Attapulgite is for symptomatic relief only and does not treat the underlying cause of diarrhea.
* If diarrhea persists for more than 48 hours, or if fever, severe abdominal pain, or blood in the stool occurs, discontinue use and seek medical attention.
* Adequate fluid and electrolyte replacement is crucial in the management of diarrhea, especially in children.
