# Attapulgite
## Overview
Attapulgite is an adsorbent used for the symptomatic treatment of diarrhea. It works by absorbing excess water and toxins in the gastrointestinal tract.
## Primary Indications
Symptomatic treatment of acute diarrhea.
## Adult Dosing
* **Suspension:** 7.5 mL to 15 mL after each bowel movement, not to exceed 10 doses (75 mL) in 24 hours.
* **Tablets:** 2 tablets (1 g) after each bowel movement, not to exceed 10 tablets (5 g) in 24 hours.
## Pediatric Dosing
* **Children 3-6 years:** 7.5 mL of suspension after each bowel movement. Not to exceed 5 doses (37.5 mL) in 24 hours.
* **Children 6-12 years:** 15 mL of suspension after each bowel movement. Not to exceed 5 doses (75 mL) in 24 hours.
* **Children 3-6 years:** 1 tablet (500 mg) after each bowel movement. Not to exceed 5 tablets (2.5 g) in 24 hours.
* **Children 6-12 years:** 2 tablets (1 g) after each bowel movement. Not to exceed 5 tablets (5 g) in 24 hours.
* **Children <3 years:** Dosing not established.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment. However, caution should be exercised, especially in elderly patients or those with significant gastrointestinal motility issues.
## Contraindications
* Known hypersensitivity to attapulgite.
* Concurrent use with other medications, as it can impair their absorption.
## Adverse Effects
Constipation is the most common adverse effect. Others may include abdominal discomfort or nausea.
## Key Drug Interactions
Attapulgite can bind to and reduce the absorption of many oral medications. Administer other medications at least 2 hours before or 4-6 hours after attapulgite.
## Monitoring
Monitor for relief of diarrhea symptoms. Monitor for signs of constipation.
## Clinical Pearls
* Effectiveness is primarily symptomatic; does not treat the underlying cause of diarrhea.
* Adequate fluid and electrolyte replacement is crucial when treating diarrhea.
* Discontinue if diarrhea persists for more than 48 hours or if fever, blood, or mucus in the stool is present.
