# Attapulgite
## Overview
Attapulgite is a naturally occurring hydrated magnesium aluminum silicate clay. It acts as an adsorbent in the gastrointestinal tract, binding to toxins and bacteria.
## Primary Indications
* Symptomatic treatment of acute diarrhea.
* Adjunctive treatment for certain toxin ingestions.
## Adult Dosing
* **Diarrhea:** 1.5 g to 3 g orally every 2 hours as needed, up to 12 g daily. Some formulations may allow for higher doses, consult specific product labeling.
## Pediatric Dosing
* **Diarrhea:**
* Children 6-12 years: 0.75 g to 1.5 g orally every 2 hours as needed, up to 6 g daily.
* Children 3-6 years: 0.75 g orally every 2 hours as needed, up to 3 g daily.
* Use in children under 3 years is generally not recommended due to risk of fecal impaction and potential for unmonitored dehydration.
## Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to attapulgite.
* History of intestinal obstruction.
* Use with caution in patients with severe constipation.
## Adverse Effects
* Constipation (most common).
* Abdominal discomfort.
* Fecal impaction (rare, particularly in children with reduced fluid intake).
## Key Drug Interactions
* **Adsorption:** Attapulgite can adsorb other orally administered medications, potentially reducing their absorption and efficacy. Administer other medications at least 2 hours before or after attapulgite. This is particularly important for antibiotics, cardiac glycosides, and anticonvulsants.
## Monitoring
* Monitor for signs and symptoms of constipation or fecal impaction.
* Assess for adequate hydration, especially in pediatric patients.
* Monitor for resolution of diarrhea symptoms.
## Clinical Pearls
* Attapulgite is a non-systemic medication and has a low risk of serious adverse effects when used appropriately.
* It is not effective for treating infectious diarrhea caused by invasive bacteria or toxins that do not bind to the clay.
* Adequate fluid intake is crucial to prevent constipation and dehydration.
