## Atorvastatin
### Overview
Atorvastatin is a statin medication used to lower cholesterol levels and reduce the risk of cardiovascular events. It works by inhibiting HMG-CoA reductase, an enzyme involved in cholesterol synthesis in the liver.
### Primary Indications
* Hyperlipidemia (primary hypercholesterolemia, mixed dyslipidemia)
* Prevention of cardiovascular disease in patients with risk factors
### Adult Dosing
* **Hyperlipidemia:** Usual starting dose is 10-20 mg once daily. Maintenance dose range is 10-80 mg once daily.
* **Cardiovascular disease prevention:** Usual dose is 10-80 mg once daily.
### Pediatric Dosing
* **Ages 10-17 years with heterozygous familial hypercholesterolemia:** 10 mg once daily, may increase to 20 mg once daily.
### Dose Adjustments
* **Renal Impairment:** No dose adjustment necessary.
* **Hepatic Impairment:** Use with caution; initiate at lower doses and monitor liver function closely. Avoid in active liver disease.
### Contraindications
* Active liver disease or unexplained persistent elevations of serum transaminases.
* Hypersensitivity to atorvastatin or any component of the formulation.
* Pregnancy and breastfeeding.
### Adverse Effects
* Common: Myalgia, arthralgia, diarrhea, nausea, headache.
* Serious: Rhabdomyolysis, hepatotoxicity, pancreatitis, peripheral neuropathy, memory impairment.
### Key Drug Interactions
* **CYP3A4 Inhibitors (e.g., itraconazole, ketoconazole, clarithromycin, protease inhibitors):** Increased risk of myopathy and rhabdomyolysis. Use caution and consider dose reduction.
* **CYP3A4 Inducers (e.g., rifampin):** May decrease atorvastatin levels.
* **Colchicine:** Increased risk of myopathy and rhabdomyolysis.
* **Niacin, Gemfibrozil, other fibrates:** Increased risk of myopathy and rhabdomyolysis.
* **Cyclosporine, Digoxin, Oral contraceptives:** Increased plasma concentrations.
### Monitoring
* Liver function tests (ALT, AST) at baseline and as clinically indicated.
* Lipid panel (total cholesterol, LDL-C, HDL-C, triglycerides) at baseline and typically 4-12 weeks after initiation or dose adjustment, then periodically.
* Monitor for signs and symptoms of muscle pain, tenderness, or weakness (myopathy).
### Clinical Pearls
* Atorvastatin is generally taken once daily, with or without food, at any time of day.
* Risk of myopathy is increased with higher doses, concomitant use of other medications affecting lipid metabolism, and in patients with risk factors for muscle disease.
* Advise patients to report unexplained muscle pain, tenderness, or weakness, especially if accompanied by malaise or fever.
* Consider lower doses in patients taking potent CYP3A4 inhibitors.
