# Attapulgite
## Overview
Attapulgite is an adsorbent antidiarrheal agent. It works by absorbing excess water and toxins in the gastrointestinal tract, helping to solidify stools.
## Primary Indications
* Symptomatic relief of acute nonspecific diarrhea.
## Adult Dosing
* **Oral suspension:** 10.2 g to 15.4 g taken orally as needed after each bowel movement, not to exceed 76.8 g per 24 hours.
* **Tablets:** 1.5 g taken orally every 1.5 hours until diarrhea is controlled, not to exceed 12 g per 24 hours.
## Pediatric Dosing
* **Children 6 to 12 years:**
* **Oral suspension:** 5.1 g to 7.7 g taken orally as needed after each bowel movement, not to exceed 38.4 g per 24 hours.
* **Tablets:** 0.75 g taken orally every 1.5 hours until diarrhea is controlled, not to exceed 6 g per 24 hours.
* **Children under 6 years:** Safety and efficacy not established. Consult pediatrician.
## Dose Adjustments
* No specific dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to attapulgite.
* Severe abdominal pain, nausea, vomiting, or fever, especially if accompanied by bloody stools, as these may indicate a more serious underlying condition (e.g., bacterial enteritis, ileus) for which attapulgite may not be appropriate and could mask symptoms.

* Constipation (most common)
* Abdominal discomfort
* Nausea
* Vomiting
## Key Drug Interactions
* Attapulgite can reduce the absorption of many orally administered drugs. Separate administration by at least 2 hours (before or after) attapulgite.
## Monitoring
* Monitor for improvement of diarrhea symptoms.
* Monitor for constipation; adjust dose or discontinue if severe.
* Assess for signs of dehydration, especially in children.
## Clinical Pearls
* Attapulgite is primarily for symptomatic relief of diarrhea and does not treat the underlying cause.
* If diarrhea persists for more than 48 hours, or if fever or other severe symptoms develop, discontinue use and seek medical attention.
* Ensure adequate fluid intake to prevent dehydration.
* Due to its adsorbent properties, consider the potential for drug interactions when other oral medications are being taken concurrently.
